I noticed a mistake after I submitted my enrollment application. What should I do?

Once your application is submitted you are not able to edit the application. A Provider Specialist will reach out to you if they have found any errors. If you have forgotten any or need to update information, you can submit a Change of Circumstance (CoC) request after your enrollment is approved. To submit a CoC, navigate to the CoC tab, click the Change of Circumstance button, and fill out the information. Your CoC will be reviewed and approved by a Provider Specialist and will either be Approved/Denied.
