What is an Affiliation?

An billing affiliation is an association of a provider with a group of providers. If Medicaid is billed through a group Federal Employer Identification Number (FEIN), the group through whom he or she plans to bill should complete a separate application and list the provider as an affiliated member, which links them to their enrollment application.

An affiliation can also be created between a Provider Enrollment Manager (PEM) and a provider that is not related to billing. This type of affiliation is allowing the PEM to enroll and manage a provider’s account.
